How Do I Get a Home Care Package? 

  1. Contact My Aged Care: Start by calling 1800 200 422 or visiting My Aged Care to arrange an assessment.
     
  2. Phone Assessment: My Aged Care will perform an initial assessment over the phone to gauge your needs.
     
  3. In-Home Assessment: An assessor will visit you at home to evaluate your daily living needs and eligibility for a Home Care Package.
     
  4. Secure Your Package: If eligible, you will be allocated a package based on your level of care. Availability may vary, and you might be placed on a waiting list.
     

Home Care Package Approved Providers 

Allied Health Centre (AHC) is an approved provider, offering tailored support plans based on your needs and goals. We assign a Home Support Partner to guide you through understanding the system, setting goals, and managing your care plan. 

Funding and Fees 

AHC works with you to structure your care within the Home Care Package budget, ensuring your needs are met within your financial means. Our goal is to support your independence and quality of life.
